Monthly Tracking Sheet

OMNI has developed Monthly Reporting Tracking Sheets to support the data collection needed to complete the Monthly Reporting Survey.

Using this tracking sheet is NOT required, and it does not need to be submitted to OMNI at any point. If you choose to use this tracking sheet, you must also complete the online survey. This tracking sheet is not a substitution for any components of the survey.

Monthly Reporting Schedule

Each month, sub-awardees are contacted to complete the monthly survey. Each month’s survey asks questions regarding the accomplishments of the previous month (e.g. the February survey will report on data from January).

*NOTE: Due to the transition of survey tools, the March 2025 survey should include data from January - February 2025.

Contacts will receive an email each month with specific instructions for survey completion.
